The
insured is utilities of Mall building such as generator, chillier, transformer,
cubicle, capacitor bank, Master processor unit of lift, hydrant pump, elevator,
escalator, pump, fire detector and PABX etc.
Utilities were maintained by own
engineer, and sub contractor. The all electrical equipment of building were
operated by own operator (technician).
There are 2 five section :
1.
Mechanical
section : hydrant pump, water supply pump, Chillier and air condition
2.
Electrical
section : electrical, Power Generator, Telephone line, fire detector, Paging
The
utilities are located in the ground floor / basement floor, excepted MPU lift
on the duct floor, fire pump, PABX, Fire detector on the 1st floor

PROTECTION
Extinguishing system
Portable fire extinguishers were
provided in the equipment and utilities area. They were conspicuously mounted
on the walls for immediate use to suppress any incipient fire. prior to the
activation of sprinkler. Total numbers of portable fire extinguishers are not
clear.
There is various size of
portable fire extinguishers (hand held type and wheeled type) that mostly
filled with Dry chemical and CO2. In the risk were installed adequate portable
fire extinguishers.
Smoke and Heat Detector.
The Insured has
installed alarm every floors of utilities area. Smoke detector and heat
detector, every 5 sq. m. with
temperature 57°
C. There are using conventional fire alarm systems with zone systems.
Hydrant and sprinkler
system
The Insured has
installed Hydrant and sprinkler system every floors. Sprinkler system was
installed every 5 sq. m.
All
fire pumps are installed in the ground floor taking suction from a hydrant
water storage tank in pump station which is automatically replenished by
grundfos pumps.
Security
There
are many people and police who specialised located in the insured area,
security guards who watch the premise for the whole days divided into three
shifts. the security is to watch people activity in the building.
Brigade
The
nearest public fire brigade is located in Lebak Bulus, Jakarta Selatan
approx. 8
km distance away. Time response for the fire engine to reach the premise is
estimated 20 minutes.
MAINTENANCE
The insured is care to good machinery such as periodically
checked the workers duty in the field due to this electrical is very danger for
equipment. The workers duties are as
follows;
1.
Daily
inspection.
The
employee is not inspect the electrical equipment or utilities before to turn on
such as :
a.
Diesel /
Generator.
Comments :
We did see daily maintenance report (log book), but
we did no see checklist at the diesel
area and check list about voltage and
current at the generator control room)
b.
Transformer :
The
last of oil transformer treatment by Cv Ernest Kolega at

found the temperature of transformer no
2 was signed 75°
C and we checked with my infrared
approx. 90°C and the room temperature was very hot
condition

Preventive
maintenance: Preventive maintenance is weekly, monthly, half year job duty as
follows :
a.
Engine system.
Radiator
maintain, engine maintain, generator maintain, cleaning room, cleaning panel
control.
Comments :
We did maintenance report sheet, log sheet of generator set.
The housekeeping in the Generator area is considered fair

Electrical Panel

Escalator / elevator system / Lift
Comments :
The escalator and Elevator were
maintain by PT Pilar Utama as sub contractor in charge for maintenance this
equipment such as for general cleaning & M/R condition, contactors, control
panel equipment, lubricant of brake plunger lever pin, brake slip distance
& brake torque, motor drive, etc. (please see elevator maintenance schedule
list)
d.
Fire fighting
system
We
found information from Mr. Surya W as Telecommunication engineer that Sprinkler
head, fire bell, fire break glass, ceiling speaker, smoke and head detector
etc were checked regularly but they
haven’t equipment checklist.
e. Chillier system
Chilled
water in / out, discharge pressure, condenser, suction, etc. were maintain by
Pt. Sampurna air condition.(please see
Chillier check list)
f Pump
The
person in charge was saw us the maintenance sheet in blank, we didn’t
receipt maintenance sheet report were fill in.

Spare part
maintenance
We
didn’t see Spare part maintenance report
are carried out periodically with regular interval such as : Oil / fuel filter,
carbon brush, etc. excepted the chillier
section

didn’t see Over haul report or schedule
list.
FERILS OTHER THAN FIRE
Explosion :
Low- not exposed.
Lightning :
Medium- the area is widely opened and the lightning conductor has been
installed at beam, it is not
clear about grounding resistance. And it is not installed arrester
Flood : Medium- the equipment is ground and many wall were leakage due to
water ground.
RSMD :
Medium- the risk is at the expose area
Burglary : Low- there are 24 hours guard
system by the security.
Impact : low – not available transport
are load / unload truck.

In the even of damage happened, it could
damages the part of the insured content.
The PML is equal to 40 % of TSI.
RECOMMENDATIONS :
1.
The
insured must have a principles are successful operation and good maintenance is
closely tied together. The primary purpose of good maintenance is to obtain
safe, reliable, and economical operation. Competent operation requires close
attention and good judgement of engine performance to recognise what
maintenance to be done. And The maintenance must be planned to coincide with
reduced production schedule. A well-organised plan and efficient mechanics are
essential because time is often limit It
is important to keep accurate records to repairs made, parts used, and notes
indicating parts that will require attention at next shutdown. An inventory record of repair parts is useful
in assuring that parts are available when needed. (It is must be make
maintenance schedule / report list each section )
2.
A
maintenance service should be provided for periodical round at the equipment as
specially in the generator area (It must be make checklist).
3.
Utmost attention should be paid to good house keeping
such as periodical removal of dust / waste / unused goods / packing material
scattered / oil leaking on floor of utilities area either by burning on site at
a safe location or removal from site and clean-up of side at least once a day.
4.
It
is recommended to installed air
condition / air fan in the transformer room area and Control panel room near
transformer area immediately due to very
hot.
5.
We
noted that the Transformer power no 2 has
temperature 75° C due to
over load power. It is recommended to decrease the power and checked the
transformer condition.
6.
Bound
wall should be provided below the diesel fuel tank near generator room area.
7.
We
recommend to every circumstance that needs covering to the MCB (on the panel
box), the cover should not be combustible one. The electrical panel must be
enclosed to avoid dust accumulated on it. It is
recommended to Clean the electric panel
due to dust accumulated and the electrical panel must checked regularly due to
prevent short circuit and over current / voltage.
8.
The
electrical cables must far from lightning conductor and to ensure that the
incoming / out going communication line, electrical line, machinery, flammable
tank and lightning conductor must be have grounding resistance below 0.5
ohm. And it must be installed Arrester
as specially at the incoming telephone line due to protect electric static, Over
current / over voltage and lightning.
