DESCRIPTION
The insured is
a local company. The risk insured is Gas Industry factory for local market (100%) as specially for Oxygen, Argon, Carbon
dioxide, Nitrogen, The mayor of product is mass product

premises was managed by own Management
with minimum fire fighting standard. They have portable fire extinguisher
facilities
The insured
premise is located in Pulo Gadung Industrial area, The insured premise is
approx. 2500 m from Pulo Gadung bus station area, which is ten minutes to
drive. The insured is easily accessible from all areas with Pulo Kambing road
nearby. It occupies approx. 24,000 sq. m. of land area, with approx. 19,000 sq.
m of
building area, as follows :

Comments :
The lay-out of plants generally in different roof (production,
filling, office, workshop building have distance approx. 6 meters distance with
the high buildings more than 8 meters and any building in one roof such as ring
spinning, waving and warehouse building..
Building inside the premise are closely arranged, some are adjacent
one to the other, How ever, the whole premises is accessible by fire truck for
fire fighting purpose. Drive ways inside the premises are 5 to 6 m wide, with
asphalt and con concrete pavement.
The insured premise is
built on the site–land that is approx. 50 cm higher than the internal road or
surrounding and every front entrance located gutter as drainage, it is
considered not effecting the premise with regard to the flood exposure. At the
time of our survey, we did not see any abnormal condition in vicinity at the
surrounding that would expose to the premise.
The insured stopped production since December 2008 and bought gas from another company for supply
customer

Protected by: The site has been fenced in completely by metal
lattice fenced with height 2 metres. All gates are closed or manned at all

via a paved road through a main gate. The Internal roads are paved and well
laid out. All parts of the plant are easily accessible. There area one access way to the plant, through the main
gate is guarded by security guards.

The raw materials are stored in 15 units of silos,
which have excellent construction and lay out.
Comments: Its hazard rating can be considered normal
Stock types are accessories of car
i.e.: rearview mirror, lamp, steering wheel padlock, muffler etc. No adequate information regarding stocks
quantity due to insured had no booking system for monitoring stocks inventory
in site.
Finishing goods stock : PPMF : 85 tons
Finished goods are stored in warehouse building, which
are adjoining to production building and utility building. Some of the finished goods are piled in high
pilling.
Stock types are accessories of car
i.e.: rearview mirror, lamp, steering wheel padlock, muffler etc. No adequate information regarding stocks
quantity due to insured had no booking system for monitoring stocks inventory
in site.

We
watch the warehouse heavily occupied, in stairway stocks also stacked. Many of
unused plastic and carton box found everywhere.

Unfortunately no adequate “No Smoking” signs installed by insured.

- The
finished products are stored temporarily in the end of each production line.
There was no abnormal placement in vicinity.
- They are placed were noted on floor. The arrangement was considered not in order
but we were advised that it was because of that time, they were on
re-arrangement in order to get tidy and neat placement.

nearest production building and warehouse and they are not bounded

team within the organization. They
have regularly training for safety skill and sometime send some employees to
pertamina for training.

All officers are expected capable to use the hand held extinguisher,
fire fighting procedure including evacuation procedure.

fire team who has conducted fire drill and training regularly. The team is
expected as the frontiest persons to anticipate fire before the fire brigade.

Portable fire extinguishers
were provided in all common areas (corridor) throughout the building. They were
conspicuously mounted on the walls or building columns for immediate use to
suppress any incipient fire, prior to the activation of sprinkler. Some of portable extinguisher were collected near
doors / gate.
There is various size of portable fire extinguishers
(hand held type and wheeled type) that mostly filled with Dry chemical and CO2.
In the raw material / finishing goods warehouse, and production area was not
installed adequate portable fire extinguishers (total number of portable
extinguishers were not in conformity with area Total numbers of portable fire
extinguishers are not clear.).
Fire extinguisher placed in
strategic places surrounding the building. In every access of
stairway, there is plan layout with pictures of fire extinguisher placed. It
also explain emergency path for emergency case.

The Insured has installed
alarm every floors. Smoke detector and heat detector (NATIONAL) every 5 sq. m.
with temperature 135° F (57° C). There are using conventional fire alarm systems for public
area, corridor, and addressable for room units.
Push –button alarm is also
available, It was installed in every
building near entrance doors, Both automatically and manual alarm system are
addressable.
We tested to the work
of alarm and it was provide good result.
- Remarks :
Water is supplied by seven deep wells each with
capacity of 350 l/min (100m deep), Pumped by 10 Hp pump. There also a connected
to the town main system for drinking water, which is kept in reinforced
concrete water tower tank 500 cu. m. capacity
All fire pumps are installed in the ground floor
taking suction from a 1,000 cu. m, 308 cu. m. and 250 cu. m hydrant water
storage tank in pump station which is automatically replenished by deep well
pumps.
Hydrant system is
reportedly fed by pump station is located in the rear of building area for consumption of all
building.
We
checked that there are 3 location of the
pump station with the same characterises
and it was interconnected, the specification as follows :

unit of Jockey pumps are also installed in each fire pump system to pressurise
the sprinkler systems and hydrant system with electric motor driven serro fire
pump 4 cu. m per minutes, 2.2 KWATT, Head: 175 m, 2900 Rpm, with system auto
power 6 s/d 7 bar off.
The insured have swimming pool with capacity is 500 m3, which it is
not connected with fire water.
Comment s :
The whole area is covered by hydrant system. Main
Hydrant pipe is 8” in diameter which branches of 6” diameter, every hydrant
outlet is equipped with nozzle and a 20 m or 30 m hose.
2 unit of Siamese connection (breaching inlets) with 2.5 inch pipe
for the fire pumps are installed outside the building for easy access by fire
brigade during an emergency.
We carried out a test during our inspection, and noted
that operation were competent. Water pressure appeared to adequate (Hydrant test result : OK it was visually satisfactory))
Water from PDAM is flown by pump into water tank. It can be
considered to be adequate for the entire premises
EXTRA PERILS

Comments :
We consider that potential problems from the said labours
are low because they have good relationship both between labour to labour and
labour to management. The management
also takes care of labour’s welfare

Comments:

The electric supplied from PLN use for lightning, packing, and in
emergency situation.

The generators set use for all operations in the plant, mainly for
production process. Three of six generators set available use for full
operation, and the rest are always standby as a reserve for in the case of the
plant needs more capacity.
- The electrical wiring was considered to be
in generally acceptable condition throughout the production areas with most of
the power cables being in PVC conduit. Power distribution cables in production
areas were noted as being in generally fair condition.

There are one steam production
natural gas fire boiler in the mechanical pad section. The boilers are
“STANDARD KESSEL” with volume 8000 kg/hours, pressure = 10 kg/cm2, 1993. With
good water purification and all of the proper safety controls are provided,
otherwise the insured boiler operators have certificates.

Welding is rarely carried out and formal welding permit procedure is
in place. The permit form was not observed but fire watch is provided during
welding operation.
Compressor :
In our resurvey visit to the risk location, we see
some compressors which are all in fair condition. There is many unit compressor that is used in production process with
many brand.

500 HP, with 10 kg/cm2 pressure, and 2
units of Broom Wade compressor with 100 HP..
Chillier :
All Production process areas are air-conditioned and supplied from
air conditioning stations which are connected to chillier at utility Building.
PT1 is supplied by 2 x 780 TR of TRANE chillers (both Chillers are in operation) PT2 has 3 x 500 TR chillers (two chillers are in
operation, one is stand by)
There is 4 unit chillies, 1
unit 800 RT Trane, 2 units 400 RT Sulzer and 1 unit 600 RT sulzer

interdependence towards other flour mills.
Bottlenecks
From generator
sets view point; we thought that it is not a bottleneck situation to be
necessary anticipated. So far, there is
no problem in that case. The port is
only used for receiving raw material they need, and it is not a port for public
us. All of activities on the port are under the insured’s control. Therefore, no specific bottlenecks are
anticipated in respect of the environmental issues.

Buildings have a
replacement period of about 2 years in the big damage.

- There
are about 30 peoples who specialised located in the premises area, security
guards who watch the premise for the whole days divided into 4 group for three
shifts. The Insured have one the guard post is to watch people go in and out
whole area It was installed sport light
every corner of plant and transceiver communication. The Insured have good
relation with local army.
- The
members carry out regular patrols using a clocking system. Security post are
distributed through the premise with distance one to another’s, there are 7
post unit
- M.P.L. / A.S. estimation : There is no value
breakdown available but we assumed that the main building is full machinery and including raw
material, finished goods building. The
PML could reach 90%.
